*An employer who enrolls in RealTime Telemed between April 1, 2020 and July 31, 2020 shall pay four dollars ($4.00) per employee per month (“PEPM”) for ninety (90) days from the effective date of the employer’s enrollment. After that ninety (90) day period has concluded, the employer will be charged five dollars and twenty-five cents ($5.25) PEPM until RealTime Telemed services are terminated. Employer will be billed monthly based on all employees passed on file to MD Live.
